Floors in prep area noted in need of repair. Floors are deteriorating so badly that they are also likely a hazard for employees. Ceiling tiles noted missing or out of place. Also the air return vent covers noted in need of repair near the ware-washing area.

IN-USE UTENSILS

Small black bowls are being used as scoops in the burger seasoning and the seasoning called ''rusty bucket''. All in-use utensils must have handles and be stored in such a manner that the handle does not touch the food product.

FOOD CONTACT SURFACES - DESIGN

Cutting board on the prep cooler with lettuce coleslaw etc. noted not in good repair. Noted with staining and deep grooves that do not allow for proper cleaning or sanitization.

EMPLOYEE PRACTICES

A personal beverage was observed in an improper location and container. An energy drink can was stored on the rear prep counter next to the burger patty press. Personal beverages must be stored away from food preparation and food-contact surfaces and maintained in a closed container designed to prevent contamination. Examples of unapproved beverage containers include open cans and bottles that require contact with the drinking surface such as screw-top bottles.

Employee toilet room door noted not self closing.

VENTILATION HOOD SYSTEM

Hood filters noted not properly in place. There are several gaps noted between hood filters which presents a fire hazard and does not properly trap grease.

NON FOOD CONTACT SURFACES - CLEAN

Outside of rolling flour bin noted not clean. Also the exteriors of equipment and cooler door gaskets noted in need of cleaning.

HANDWASHING LAVATORIES AND SUPPLIES Critical

A hand drying device was noted not provided at the hand washing lavatory in the kitchen. Paper towels or batteries were replaced to correct the violation.
